Select a County or Community GENERAL AMERICAN COMMUNITY SURVEY: Allows reader to select state, obtain census data, and make comparisons with previous years (part of American FactFinder). YOUTH RISK BEHAVIOR SURVEILLANCE SYSTEM (YRBSS), Supported By The Centers For Disease Control And Prevention, US DHHS. Developed in 1990 to track health behaviors and risk factors that contribute to death, disability and social problems among children, youth, and adults in the US. Public health focus. Data are collected on tobacco use, dietary behaviors, physical activity, alcohol and other drug use, sexual behaviors and sexually transmitted diseases, and behaviors that contribute to unintentional injuries and violence. Data are collected through national, state and school-based surveys every 2 years. STATE-SPECIFIC Colorado Health Information Data Sets. Allows access to local-level data on variety of issues and indicators to determine health status of a neighborhood, community, county or region. ILLINOIS Project for Local Assessment of Needs (IPLAN). Contains 102 indicators for county-level assessment of health needs; 39 indicators are available at the community level. KENTUCKY Cabinet for Health and Family Services Surveillance and Health Data NEW YORK PRISMS: Prevention Risk Indicator Services Monitoring System. Information on 62 risk indicators and ten consequences indicators is provided in tables organized under three general domains for each county: community risk, youth risk and early consequences of alcohol and substance abuse (ASA). Risk indicators are individual measures of a risk factor that are combined into risk constructs. Consequences indicators are measures of problems youth are experiencing as a result of ASA.
