National and State Data You can find additional information through our Federal Government resources. US GENERAL: Community Health Indicators Project, Public Health Foundation. The Community Health Status Indicators (CHSI) project has produced a county-specific report of community health status for local jurisdictions across the United States . This collaborative activity of the Association of State and Territorial Health Officials, the National Association of County and City Health Officials and the Public Health Foundation is being funded by the Health Resources and Services Administration (HRSA), United States Department of Health and Human Services. The goal of the CHSI project was to provide important health and health-related data, presented in a way that makes them useful to communities. US CENTERS FOR DISEASE CONTROL AND PREVENTION: State-based Healthy People 2010 Data from “CDC Wonder” supported by the Centers for Disease Control and Prevention, US DHHS.
